2. Services Description

PrimeDraw Specimens provides mobile phlebotomy and laboratory specimen collection services. Our Services include, but are not limited to:

• Blood specimen collection at patient homes, workplaces, or care facilities
• Laboratory testing coordination and specimen transport
• Patient screening and intake services
• Custom laboratory panel testing
• COVID-19 and influenza testing
• Specialty testing services

All specimen collection is performed by certified and licensed phlebotomists. Laboratory analysis is conducted by certified third-party laboratories.

3. User Responsibilities

By using our Services, you agree to:

• Provide accurate, complete, and current information when scheduling appointments or submitting forms
• Disclose all relevant medical history, medications, allergies, and conditions to the phlebotomist prior to specimen collection
• Be present at the scheduled appointment location at the agreed-upon time
• Provide a safe and accessible environment for our phlebotomists to perform specimen collection
• Ensure that a qualified adult is present during specimen collection for minor patients
• Promptly notify us of any changes to your scheduled appointment
• Use our Website only for lawful purposes and in accordance with these Terms

4. Appointment Scheduling & Cancellation

Appointments may be scheduled through our Website, by phone at 951-667-3879, or by email at info@primedrawspecimens.com.

We request at least 24 hours' notice for appointment cancellations or rescheduling. Late cancellations or no-shows may be subject to a cancellation fee. We reserve the right to require prepayment for future appointments following a no-show.

We make every effort to arrive at appointments on time. However, we are not responsible for delays caused by circumstances beyond our control, including but not limited to traffic conditions, weather events, or other unforeseen circumstances.

5. Payment & Insurance

We accept most major PPO insurance plans. It is your responsibility to verify your insurance coverage and benefits prior to receiving Services. Any co-pays, deductibles, or non-covered services are your financial responsibility.

For cash-pay patients, payment is due at the time of service unless prior arrangements have been made. We accept cash, credit cards, and debit cards.

You are responsible for any charges incurred for Services provided, regardless of insurance coverage determinations made after the service date.

6. Intellectual Property Rights

All content on our Website, including but not limited to text, graphics, logos, images, icons, audio clips, digital downloads, data compilations, and software, is the property of PrimeDraw Specimens or its content suppliers and is protected by United States and international copyright, trademark, and other intellectual property laws.
